Pre-Employment Affidavit – Required for All Private Schools:

Private schools must use the for all candidates for teaching positions. The below form below meets the requirements of TEC §22A.055.
 
 
The affidavit may be completed as a notarized affidavit or unsworn declaration (non-notarized). Educational entities may determine which version to use. Additionally, educational entities may rekey the form to integrate with their own hiring systems.

Logging in to the Do Not Hire List – Required for TEFA Participation

  1. Please check your school’s entry at www.tepsac.org under “search schools” and make sure that the person requesting access to the list is the person listed in the contact email.  If this information needs to be changed, please email Laura@TexasPrivateSchools.org.
  2. Log in to TEAL at https://tealprod.tea.state.tx.us/
  3. On the left, select Edit My User Information.
  4. Near the bottom of your TEAL profile, click the Manage Approver Status link.
  5. Select Request Approver Status.
  6. From the drop-down menu, select the type of approver role needed.
  7. In the field for Organizations whose user requests you will approve, enter the organization name or ID. (As you type, matching organizations appear. Select your organization from the list, and then click Add.)
  8. If your organization is not listed, you will need to contact the TEA help desk for your organization to be added to TEAL. (Response time averages around 24 hours)
  9. When you finish entering all information, click Submit. A message is displayed indicating that your request was successfully submitted.
  10. Once you are approved with access to the list, then you will upload Last Name, First Name, Social Security Number, and Date of Birth of employees to see if any matches come up with a person on the list.
  11. A TEA help desk is standing by.  Please contact them between the hours of 8am and 1pm at 512-936-8400 or submit a help ticket as a School District/Charter School at Fingerprinting and Registry Support Help Desk Ticket.

 

Misconduct Reporting – Required for All Private Schools

Private school heads must report within 48 hours if there is evidence that an employee or contractor had inappropriate relationships, communications, or boundary violations with students. The reporting process is as follows and may be found on the TEA website:
 
Written misconduct reports must be submitted via the TEA portal through the TEAL login outlined above.  More information may be found here. The main reasons that reports fail to upload are that the reporter is not using Google Chrome and/or needs to clear the computer cache. After that, the report will upload.
 
The report must contain the following:
  1. A summary of the factual circumstances underlying the report.
  2. The name of the individual reported and any aliases.
  3. The reported individual’s certificate number, if any, or social security number and date of birth.
  4. The reported individual’s last known mailing address and home and daytime phone numbers.
  5. The names and any available contact information of any alleged victims.
  6. The names and any available contact information of any relevant witnesses to the circumstances requiring the report.
  7. The employment status of the individual, including any information about termination or notice of resignation.
  8. The name and contact information of the law enforcement or other agency involved in the review of allegations.
